If you've recently been accepted to the JPWA you'll need to fill these forms out and get them submitted to [email protected] as soon as possible. As a class begins to fill up it is the receiving of your completed paperwork that dictates that you have successfully secured your spot in the class, not the time of your application. Please don't delay returning your paperwork as the class may fill up by the time you return it if you wait for a long period of time.
The preferred method is to print them, fill them out, and scan them. However, if you need to send a photograph of the pages this CAN work if the pages are laid flat and are easily readable.
